You are kindly requested to be as complete as possible and do not forget the signature and picture page. Because of our European Branch membership badge we need your signature and a good picture.

You are also requested to return the form to me before the first of October 2007.This gives me the possibility to update the administration before our annual seminar in Stockholm.
